The Community Health Representative Program is a contract Program through I.H.S. The CHR Program for the Delaware Nation uses I.H.S. recommendations to set up and implement the CHR Program on Health promotion and especially disease prevention. The CHR program is funded to the tribe to be able to offer medically related services to Native Americans within our service area, with priority given to the Delaware Nation members.

Delaware Nation Special Diabetes Program for Indians (SDPI)

All the following criteria must be met in order to receive assistance: (Proof of each is required)

  • Delaware tribal member or non-indigenous Native American
  • Reside within our service area
  • Do not already benefit from another tribal SDPI
  • Have been diagnosed with diabetes mellitus

Please fill out the application completely and sign and date it. A separate application for each must be filled out and approved for further assistance. Each application is provided separately below.

“Prescription from Doctor” means that you must either:
Submit a “Health Summary” from the IHS facility you use showing your diabetes mellitus
(OR)
Have your primary care physician write on a prescription pad stating that you have been diagnosed with diabetes mellitus and that he/she recommends you have good walking shoes, dentures, and/or eyeglasses.
