Competed applications must be submitted within the first 6 weeks from the semester start date.

• Must be an enrolled Delaware Nation Citizen
• Must complete a Higher Education application along with required supportive documentation
• Must submit a copy of Delaware Nation Enrollment card
• Must submit a letter of intent (this letter must be written by the applicant)
• Must submit a letter of admission from the college
• Must submit a copy of class schedule
• Must submit a college/High School transcript
• Must submit a copy of student aid report (SAR)
• Must complete a financial need analysis (this form is in the application packet and it is to be completed by the financial aid office of the college they will attend)

• Must be an enrolled Delaware Nation Citizen
• Must submit a Higher Education application before CFA application is considered
• Must submit an application each semester funds are needed.
• Must be enrolled in 12 credit hours and maintain a 2.5 GPA per semester

• Must be enrolled with the Delaware Nation
• High School & College Senior must have at least a 2.5 overall GPA
• APPLICATION DEADLINE: 30 days before graduation date.
• Applications will be reviewed for completeness and eligibility. Application approvals will be done in writing by the Delaware Nation Education Office.
• All checks will be made payable and mailed to the vendor.
• NO REIMBURSEMENTS will be made for purchases made prior to this application.

• Complete application with supportive documents.
• Copy of CDIB card
• Invoices MUST be submitted with application.
• Copy of high school transcript and current report card. Must reflect attendance.
• Invoices for senior pictures, graduation announcements and cap & gown must be submitted with complete application.
• A typed essay stating:
  • The name of applicant and what school student attends
  • Age of student
  • List type of extracurricular activities the student is involved in such as:
    • Athletics
    • Culture, Academic and/or Art clubs
    • Volunteer work or Community Service
    • Internships
    • Employment
  • The future plans the applicant has after graduation.

• High School $60

Requirements:

• Submit a completed application
• Submit a copy of high school or college diploma

• January 1st for fall semester
• June 1st for spring semester
• August 31st cut-off date for late fall and spring requests for the whole academic school year

• Must be an enrolled Delaware Nation citizen in 5th-12th grades
• Must submit a complete application
• Must write a brief essay stating name, age, grade and that all original receipts will be returned back to the education department
• Must submit a copy of report card with semester grades only
• Must submit receipts each semester in order to qualify for the following semester
• No more than 5 absences per semester

• Must be an enrolled Delaware Nation citizen
• Must submit an application with supportive documents
• Must maintain a 2.5 GPA per semester
• Must submit grades at end of each semester
• Must notify Social Services immediately if withdrawal from any class is submitted to college

• Must be an enrolled Delaware Nation citizen
• Must complete an application and submit all supportive documentation
• Must submit letter of intent
• Must submit letter of admission as proof of enrollment
• Must submit tuition cost printout by the college financial office
• Students are required to notify the Social Services Department if and when courses are dropped. If student drops a course or does not complete a course and a refund is not issued from school, the student will be required to pay back all funds used before being eligible to participate in program again.

• Must be an enrolled Delaware Nation citizen
• Must submit a completed application along with supportive documentation
• Must submit a letter from new employer on their letterhead indicating:
  • date of hire
  • number of working days per week
  • hourly rate for full time status
  • type of pay period (i.e., weekly, bi-weekly, monthly, etc.)
  • date of first paycheck received
  • an attached job description on company letterhead
• If you require special tools or uniforms you must submit an invoice from where you will purchase your items. A letter from the employer may be necessary to determine need. Checks will ONLY be made to vendor.
• W-9 Form must be filled out and turned in with this application

Tribal Citizens:

• Food $150
• Utilities up to $150 Must attach utility bill.  No minors can apply for utility assistance
• Glasses $200 for tribal citizens (0-59)

Tribal Citizens (60 and older):

• Glasses up to $400 every 2 years
• Major Dental up to $500 every year
• Dentures up to $3,000 every 5 years
• Hearing Aids up to $3,000 every 5 years

ASSISTANCE:

The following benefits are for all tribal citizens.

• Food $150

• • Unapproved grocery purchases include: makeup, jewelry, perfume, hair products, tobacco and alcohol products. Applicants are required to submit receipt for items purchased.

• Utilities $150

• • Tribal citizen must be 18 years old or older to receive utility assistance and attach a utility bill in the tribal citizen’s name. All cut off notices for utilities will take 7-10 business days before a check will be issued to utility vendor. Delaware Nation is not responsible for late fees or reconnect fees.

HEALTH:

•  Glasses up to $200 for tribal citizens (0-59)

ELDER HEALTH ASSISTANCE:

All applicants must be 60 years old or older at the time of request. An elder application must be submitted for assistance.

• Glasses up to $400 every 2 years
• Major Dental up to $500 every year
• Dentures up to $3,000 every 5 years
• Hearing Aids up to $3,000 every 5 years

REQUIREMENTS:

• All applications must have a bill or invoice if you apply for UTILITIES, DENTURES OR GLASSES. If not your application will be placed on pending status.
• Minors (under 18) cannot apply for utilities.
• Our office cannot rush checks for utility cut off notices.

• Head of household or spouse must be an enrolled Delaware Nation citizen
• Household cannot have received LIHEAP thru DHS or any other funding source that receives LIHEAP
• Submit a completed LIHEAP application and supportive documentation
• Provide income verification for all household members over the age of 18
• Provide a copy of CDIB and social security cards for everyone in the household • Provide a copy of current utility bill in applicant’s name

RESPONSIBILITIES

• All supportive documentation must be submitted with an application to be considered for assistance.
• Failure to report total household income may result in being disqualified from the program for one year.

• Head of household or spouse must be an enrolled Delaware Nation citizen
• Household cannot have received LIHWAP thru DHS, another tribe or any other funding source that receives LIHWAP
• Submit a completed LIHWAP application and supportive documentation
• Provide income verification for all household members over the age of 18
• Provide a copy of enrollment card (any tribe, if applicable) and social security cards for everyone in the household
• Provide a copy of current water utility bill in applicant’s name

RESPONSIBILITIES

• Enrollment cards (for any tribe, if applicable) for all household members
• Copy of Social security cards for ALL household members
• Proof of Income (last 30 days) for ALL household members over the age of 18
• Copy of water utility bill in the tribal citizen’s name

Elders have a choice of:

1. Turkeys and blankets on Thanksgiving or
2. Hams on Christmas or
3. Fans during the summer.

The CSBG gives these items on a first come first serve basis.

The youth activity is held usually the 1st or 2nd week in August during the American Indian Exposition in Anadarko, Oklahoma.  The American Indian Exposition is a six day event that is held (Monday-Saturday).   The Delaware Nation CSBG program purchases fair bracelets for enrolled Delaware children ages 3-12 years old that attend the American Indian Exposition carnival.  The bracelets are given on a first come first serve basis.

Delaware Nation provides burial assistance for enrolled tribal citizens or those minors eligible to be enrolled (and not enrolled in any other tribe) up to $6,500.

• PREPAID BURIAL ASSISTANCE: A tribal citizen (age 60 or over) or diagnosed with a terminal illness, may obtain assistance to purchase an irrevocable contract burial plan with a funeral home of their choice.

• A maximum payment of $7,500 is authorized for burial assistance for deceased tribal citizens. (The burial check will be made to the funeral home selected.)
• $250 is allotted to the family for food for a memorial meal (The next of kin on the application will receive the check.)

• Must submit an invoice of funeral home costs
• Must submit a Death Certificate (Please request one for the tribe when making funeral arrangements)
• If applicable, must submit a copy of the irrevocable contract burial plan
• Eligibility must be confirmed by the Enrollment Department
• If applicable, a letter from the tribal citizen’s medical doctor stating the terminal diagnosis

• Must be an enrolled Delaware Nation citizen
• Must submit a copy of CDIB card
• Must submit a copy of prescription from the physician
• Must submit a statement from the physician, clinic or hospital explaining your injury and/or disability
• Must submit proof of income
• Must submit proof of insurance or Medicare

ATTENTION: (Applicants must have denial statements from insurance, Indian Health Services or other medical facility in order to receive services. Denial must state that prescribed medical equipment or prescriptions are not covered by insurance or Indian Health Services.)

• Child(ren) must be enrolled, eligible to be enrolled, or whose parent is enrolled with a federally recognized tribe
• Parent(s) must be working, attending college or vocational training
• Child(ren) must be ages 0-12
• Complete a Child Care application with supportive documentation
• Must provide proof of employment or class schedule of school attending
• The parent(s) must select the child care provider. The child care provider must be licensed by the state of Oklahoma and be approved or eligible to be approved by Delaware Nation

• The parent(s) is responsible for completing and submitting an annual application to this office
• Abide by the day and hours as specified in the day care plan for my child(ren)
• To sign child(ren) attendance record every month
• To never sign a blank attendance record
• To make my co-payments on time to the child care provider
• To make any payments to the care provider for days/hours over the agreed care plan
• Parent/guardian MUST notify the Delaware Nation Child Care Coordinator of:
  • Change of facility or care provider
  • If child care services are no longer needed
  • Reasons for child not attending day care
  • Change of family income
  • Change in family size
  • Change of address and phone number

If you fail to secure the documents listed below your file will be placed on pending status

• Copy of CDIB’s and/or pending enrollment letter for each child and parent/guardian
• Copy of the past month’s income to include paystubs, child support, alimony, or any other source of income
• Copy of school and/or training schedule for parent/guardian
• Updated immunization records for each applicant
• Letter from DHS stating not receiving state child care assistance
• Copy of day care provider license

• Assists to prevent the breakup of Delaware Nation enrolled children and families at risk
• Assists tribal families to find resources to retain children in the home
• Monitors state courts in child custody proceedings involving Delaware Nation enrolled children
• Assist parents to carry out court ordered obligations and treatment plans
• Provides counseling for foster children in Delaware tribal custody
• Provides counseling services to abusive and neglectful parents

• Provide protective and preventive services, which will assist family members in achieving identified goals and outcomes from their court ordered treatment plans

• Protect the rights of Delaware Nation children and families who fall under the purview of the Indian Child Welfare Act (ICWA)
• To assist families those are involved through Indian Child Welfare and or Department of Human Services
